Quality Assurance and Performance Improvement

C3’s Quality Team implements program evaluation, performance measurement, and quality improvement techniques to ensure care is delivered consistently across our network. C3 Agencies individually contribute to the broader vision and development of evolving Home and Community-Based Services quality measures.

C3 agencies utilize data-driven dashboards to manage performance and achieve nationally-recognized quality assurance standards and provider monitoring expectations.

C3’s agencies are some of the National Committee for Quality Assurance (NCQA) Case Management for LTSS® Early Adopters. In addition to NCQA, C3 is also implementing Quality Assurance and Performance Improvement based on best practices noted in multiple Nationally Recognized healthcare quality organizations including:

  • Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Measures
  • Health Outcomes Survey (HOS) Elements
  • HEDIS® Standards
  • Medicare STAR Ratings
  • National Core Indicators for Aging and Disability Services™
